Service Description
For NDIS services 1. Please provide a copy of your NDIS Plan goals. 2. Please provide the name of your NDIS Plan Managers, or 3. Please state if you are self managed, in the comments section. 4. Prior to commencement of services, an NDIS Service Agreement will need to be signed. Feel free to call me to discuss your personal circumstances.
Cancellation Policy
PLEASE NOTE NEW CANCELLATION POLICY - effective from 1 October 2024 All previous cancellation arrangements and Agreements are hereby revoked in favour of this new policy. New Policy as follows : ALL CLIENTS including NDIS client : 2 full business days notice required. A business day is defined as 9am-5pm. Note: Public Holidays are excluded as they are not business days. Cancellations that fall outside the specified limit above will incur a 100% late cancellation fee. Exceptions will be considered : if we are able to fill or reschedule your appointment. Rescheduling must fall within 4 (four) days of your original appointment date. NDIS participants - cancellation is in accordance with the latest NDIS amendments at 1 October 2024.
